A leading healthcare organisation is seeking to fill two positions for Associate Transformation Managers to support the development of Primary Care Networks. These roles focus on project management, collaboration with stakeholders, and delivering high-quality healthcare outcomes in Hillingdon. Positions are available full-time and part-time with fixed-term contracts lasting until March 2027.
Job summary
We are looking to recruit 2 Associate Transformation Managers on fixed term contracts until 31st March 2027. 1 role is full time 37.5 hours per week and the other is part time 18.75 hours per week.
The Confederation Hillingdon CIC is a collaborative organisation which works with general practices across Hillingdon with a vision to deliver Better Health through Collaborative Primary Care.
We work with general practices and other healthcare providers in Hillingdon to deliver high quality services for patients. In addition, we aim to improve care for patients by facilitating collaboration across general practice, releasing clinical time and developing the primary care workforce.
The post holder will be key in supporting the Transformation Managers in supporting the Primary Care Networks and Neighbourhoods to deliver key priorities and maximise practice income. Using achange management approach and methodologythe Associate Transformation Managers will help toidentify,planand delivertransformationinitiatives to support and enhance Neighbourhood working.
Main duties of the jobThe Confederation, Hillingdon CIC works with General Practice and other healthcare providers to deliver its vision for Hillingdon to deliver the best primary care outcomes for patients in the whole of London. We are a not for profit community interest company. The Confederation works to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We deliver excellent clinical services ourselves both at scale and complementary to General Practice. We are the Hillingdon provider representative voice for local General Practice into the wider NHS and other Partners. We are of the NHS but independent, innovative and transformational.

The duties and responsibilities will include, but will not be limited to, the following:
Neighbourhood Development
Support the Transformation Managers in the development and delivery plans for the Neighbourhood/PCNs/Practices
Support and inform the design and implementation of the Neighbourhood strategy and development
Working on various HHCP programmes within the Neighbourhoods and individual Neighbourhoods development plans.
Support the Transformation Managers to work on various Primary Care Network(PCN) programmes that support Neighbourhood working
Lead on various programmes of work such as Enhanced Services and Local Enhanced Services, ensuring that SOPs and materials to support practices and PCNs are developed.
Work with the Transformation Managers to utilise PCIT dashboards and help PCNs to monitor their achievements against contractual targets
Support the Transformation Managers to use population health data and various BI data to analyse individual neighbourhood development areas and to drive transformation and continuous improvements
Facilitate and enable transformational change and build excellent functional relationships with and between multiple stakeholders
Support PCN Managers and work alongside HR and Finance to develop the PCN workforce plans and maximising their ARRS allocation
Leading on the submission of PCN documentation to commissioners in agreement with the Clinical Directors and PCN Managers
Identify process improvement and where possible streamlined ways of working
Utilise project management and change management methodologies to manage and deliver against the key deliverables (or key performance indicators KPIs)within the project to ensure it is successful
To support the achievement of Neighbourhood objectives through effective facilitative partnership working and engagement to ensure change is embedded
To investigate best practice elsewhere, evaluate applicability to the Neighbourhoods and help to develop plans to ensure successful implementation.
Deliver real measurable results against externally set outcomes, including clinical outcome improvements, changes in where and how care is delivered, and tangible cost savings.
Provide and receive highly complex, sensitive and contentious information, presenting information, to a wide range of stakeholders in a formal setting.
Work alongside the Transformation Managers to help develop effective networking to share good practice across the Hillingdon practices/networks
Research:
Working with the Clinical Lead and the National Institute for Health Research to promote research in general practice in Hillingdon to help increase uptake of research activity in the Borough.
Being the key point of contact between the national team and Hillingdon practices; disseminating information, co-ordinating responses and reporting
Utilising EMIS reporting solutions to undertake searches on behalf of practices to increase research uptake and increase practice income opportunities
Attending meetings and forums with Hillingdon practices to inform them about research opportunities
Creating strong relationships with practices and create promotional materials such as our Research Newsletter
Liaise and coordinate with the Finance department to ensure practices and care homes are accurately paid for all research activity.
Communication and Partnerships:
Promote a positive image of the Confederation Services both externally and internally.
Assist with the production of accurate information for Confederation, on Confederation and CCG websites as requested.
Play a key role in presenting at the HHCP induction programme to inform new starters of the Confederation and Neighbourhood programme of work.
Participate in the induction and training of new members of staff and contribute to the Confederation development.

Foster and maintain strong links with all services across the PCN and neighbourhood networks.
Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.
Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs)
Can recognise personal limitations and refer to more appropriate colleague(s) when necessary.
Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to:
o Patients and their representatives.
o GP, nurses and other practice staff.
o Other healthcare professionals includingcommunity pharmacists, pharmacy technicians,social prescribers, first contact physiotherapists, physicians associate and paramedics.
o Community pharmacists and support staff
o Other members of the medicines management (MM) team including pharmacists, technicians and dieticians
o Community nurses and other allied health professionals
o Hospital staff
Professional Development:
Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
Involved in one to one meetings with line manager monthly to discuss targets and outcomes achieved.
Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
Review yearly progress and develop clear plans to achieve results within priorities set by others.
Health and Safety/Risk Management:
The post-holder must comply at all times with all local Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).
