Job Overview

An opportunity has arisen within the CNWL NHS Foundation Trust, Community Health Services, for a Rehabilitation Assistant to join our Staying Steady, falls prevention service. Due to expanding and innovative project work, we are further resourcing our team to carry out this exciting new work. Based in Bletchley, Milton Keynes, Staying Steady is a multi-professional team undertaking multifactorial risk assessments and providing solutions such as equipment prescription, advice, signposting, and other interventions to reduce falls risks for adults in Milton Keynes.

This role offers the chance to develop assessment and treatment skills for individuals who have fallen or are at risk of falling. You will work alongside experienced professionals including Physiotherapists, Occupational Therapists, Nurses, Rehabilitation Support Workers, and Consultants to deliver coordinated assessments and goal-oriented interventions.

Interviews will be conducted face-to-face.

Main duties of the job
  1. Undertake skilled support work to aid rehabilitation and promote independence.
  2. Provide flexible therapeutic activities under professional guidance.
  3. Follow treatment plans developed by health professionals.
  4. Work independently, monitoring progress and adapting interventions as needed, including patient assessments.
  5. Carry out community interventions, primarily in patients' homes, under professional guidance.
  6. Participate in developing assessment and treatment planning skills within a competency framework.
  7. Maintain high standards of documentation and communication, including electronic systems.
  8. Lead group therapy exercises and education sessions with team support.

Detailed Job Description And Main Responsibilities
  • Work autonomously within the remit of a Rehabilitation Assistant.
  • Operate mainly alone in community settings, managing falls-related cases independently.
  • Conduct falls risk assessments at patients' homes using acquired skills and training.
  • Discuss functional goals with patients, develop, and implement action plans.
  • Develop individual programs with guidance from team professionals.
  • Organize and manage group exercise sessions and booking venues.
  • Progress treatment plans, including mobility, gait re-education, and home exercises.
  • Assess and prescribe equipment, liaising with health professionals and agencies.
  • Record and evaluate patient progress, ensuring safety and effective rehabilitation.
  • Adapt treatment programs to home environments creatively and practically.
  • Maintain accurate patient records meeting CNWL standards.
  • Provide education and support to patients, carers, and colleagues.
  • Collaborate with other agencies to support patient rehabilitation.
Person Specification
Knowledge and Skills

Essential:

  • Experience in NHS or social care settings.

Desirable:
