About the Role:

Main Responsibilities:

North High Street is a care experienced young persons service, focusing on young people aged between 16 and 25. Thisfixed term Support Worker role working at North High Street Service involves working on a rolling rota coveringa variety of shift patterns including some weekends. This role is 31.5 hours per week. The Support Workerwill:

  • work as part of a team providing safe, secure, supported accommodation for homeless people.
  • support,review, monitor and evaluate through our case management system, to achieve a positive person-centred outcomefor the people we work with in line with organisational, statutory and legislativerequirements.
  • maintain and update all recording systems and documentation in line with relevantpolicies, procedures and practice guidelines, including the occupancy agreement, within requiredtimeframes.
  • provide, plan and facilitate key work meetings, key work sessions, meetings, reviews andother internal/external meetings in conjunction with the person’s support plan.

The support we provide is always person-centred, trauma-informed and wellbeing focused. As an Intensive SupportWorker, you’ll work in the community to provide a wide range of flexible intensive support to individuals toencourage independence to maintain and sustain a tenancy and adapt to community life.

Main Responsibilities:

This Intensive Support Worker role working in the Kilmarnock area combines settled housing with person-centred,strengths-based and flexible support – as much and for as long as someone wants it. There are currently twopositions available due to recent service expansions, with both roles working 35 hours and 28 hours per weekrespectively on a rota covering various shift patterns including select weekend work. This role operates acrossKilmarnock, with an office based at our Kilmarnock Whatriggs service. The Intensive Support Worker will:

  • Provide intervention and a range of supports for new referrals and existing Supported People in Blue Triangle’ssupported accommodation services and move-on accommodation.
  • To provide trauma-informed, person-centred,and wellbeing-focused support for people supported by Blue Triangle.
  • To support, review, monitor andevaluate through our SHARP monitoring tool to achieve a positive person-centered outcome for the people we workwith in line with Blue Triangle’s statutory and legislative requirements.
  • To support peopletransitioning from supported accommodation into the local community or referred from other sources, helping themon their personal pathway towards a settled tenancy, building their tenancy and life managementskills.
