Job Overview

We are seeking a dedicated and experienced professional to join our Anticoagulation Service as an Anticoagulation Practitioner. This role involves working collaboratively across all sites within Frimley Health Trust and with our partners in primary care. It offers an exciting opportunity to contribute to quality improvement initiatives and deliver high-quality, patient-centred care.

This position is open to registered nurses or pharmacists with experience in managing anticoagulation therapy. Candidates should be capable of autonomous practice and demonstrate a commitment to excellence in patient care.

Main Duties of the Job
  1. Develop the anticoagulant service at Frimley Health in conjunction with specialist nurses and the lead haematology consultant.
  2. Provide skilled and competent care to meet patients’ anticoagulation needs, involving other healthcare team members as appropriate.
  3. Ensure continuity of care.
  4. Assess, plan, and implement care for patients requiring anticoagulant therapy according to protocols.
  5. Assist in developing anticoagulant care pathways to facilitate seamless care, acting as a key worker.
  6. Ensure patient treatment and care are based on best practices and evidence as a leader in anticoagulation.
  7. Liaise with patients and GPs and organize implementation of planned actions as decided with the lead Haematology Consultant.

Person Specification
Qualifications
  • NMC registered nurse
  • Degree-level registration
  • Mentor Prep teaching qualification (ENB 998 or City & Guilds 730 or equivalent)
Desirable Qualifications
  • Recognised anticoagulation course
  • Non-medical prescribing course
Experience
  • Recent clinical experience in relevant practice area
  • Experience collaborating with a multi-professional team
Skills & Knowledge
  • Ability to train and supervise junior staff, patients, and carers
  • Ability to manage a team in the absence of the team leader
  • Ability to work independently within role boundaries
  • Participation in delegated tasks, mentoring, auditing, and clinical governance
  • Understanding of current issues in an NHS setting
Desirable Skills & Knowledge
  • Experience in practice change and project management
  • Understanding of anticoagulant therapies
