Working within the Surrey Liaison and Diversion Service, the role involves providing outreach support in the Surrey community.
Salary: GBP29,220
Hours: 35 hours per week
Duration: Permanent

Location: The main office is based in Guildford, with service coverage across Surrey including Woking, Chobham, Runnymede, Redhill, and others. The role involves blended working and some travel to visit clients, with travel expenses reimbursed. Car owner-driver preferred.
The Assertive Outreach Worker collaborates with colleagues to offer person-centred, community-based support aimed at improving health and social care outcomes and reducing offending. The role involves assessments to identify vulnerabilities such as mental health, physical health, learning disabilities, substance misuse, housing, and financial issues. Support includes signposting, referrals, appointment assistance, collaborative working, and holistic wellbeing support.
