The Role:

As a Relief Support Practitioner, your responsibilities include:

• Supporting independence at home
• Providing personal care and assistance with daily activities
• Meal preparation and nutritional support
• Offering emotional support and companionship
• Preventing hospital admissions proactively

You will work across Mobile Home Care and Responder services, with shifts including evenings, weekends, and nights, providing flexibility.
