An exciting opportunity has arisen for a Band 5 Community Nurse within the Northwest Integrated Neighbourhood Team based at Hartismere Hospital in Eye. This is a 12-month fixed-term or secondment position.
Note: This role could be classified as Band 6 depending on relevant experience, which will be assessed at interview.
The team comprises district & community nurses, assistant practitioners, generic workers, occupational therapists, physiotherapists, and social care workers. It is led by a Nursing Clinical Lead, integrated with a Therapy Clinical Lead, and managed by an INT manager. The successful candidate will work across the Northwest INT and wider community services to meet service requirements.
The team prides itself on a holistic approach to care and is continuously evolving to support NHS developments, including close collaboration with statutory, non-statutory, and voluntary services.
You will receive regular supervision, annual appraisals, and access to relevant skills development training. The role requires autonomous working on behalf of local patients.
Operational hours are from 08:00 to 18:00, covering shifts over 7 days.
Responsibilities include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multidisciplinary team meetings, PICC line care, catheterisation, equipment ordering, student support and mentoring, and involvement in link roles such as tissue viability.
We offer various training opportunities, including in-service, mandatory, and clinical skills training facilitated by practice development facilitators. The team supports professional development through forums, meetings, and training with neighboring community teams in Health and Social Care.
