With an increased focus on delivering care in the community to prevent hospital admissions and facilitate early discharge, now is an excellent time to join the Community Children's Nursing Team in Tower Hamlets.
We are seeking a motivated individual with excellent communication, interpersonal, and organizational skills to join our team. You will deliver care within an integrated community setting. This 12-month fixed term contract is to cover study leave.
Your responsibilities will include providing day-to-day evidence-based nursing care and case coordination for children and young people aged 0-18 with acute, short-, and long-term nursing needs. You will adopt a person-centered approach, focusing on the individual needs of the child and their families, encouraging active engagement to empower and foster independence whenever possible.
