Hywel Dda Health Board presents an exciting opportunity for a motivated, passionate, and experienced individual. The successful candidate will join our multidisciplinary front-of-house team, TOCALS (Transfer of Care Advice and Liaison Service), as a Clinical Lead Physiotherapist. TOCALS supports a wide variety of patients, presentations, and pathologies, including falls and frailty, neurological conditions, cardio-respiratory issues, trauma, and orthopaedic problems (including major trauma patients).
TOCALS aims to prevent unnecessary hospital admissions, promoting high-quality, person-centered care that considers the patient holistically, fostering independence and wellbeing. The service plays a crucial role in linking acute, intermediate, community, and third-sector services to ensure continuity of care and the right care in the right place at the right time.
The successful candidate will have extensive postgraduate experience across various clinical areas, applying this to diverse patient populations. They will be capable of providing highly specialist assessments focused on discharge planning, developing and implementing co-produced, personalized care plans. The post involves tracking patient journeys through hospital and into the community.
The candidate must demonstrate a passion for high-quality care and experience in service improvement aligned with national drivers. Strong leadership, organizational, and team management skills are essential, especially in fast-paced environments. The role includes supporting ongoing professional development, leading training programs, staff supervision, and annual reviews. Supporting students through Clinical Supervisor duties is also a key aspect.
