Job description

Hours: Full time - 37.5 hours per week (Shifts: 8am-4pm, 1pm-9pm, 9am-5pm; includes weekends). Job ref: 350-MHC7152655. Employer: Mersey Care NHS Foundation Trust. Employer type: NHS. Job overview

Join us at Leigh Moss Community Hub, Liverpool, as an Older Adult Community Access Support Practitioner (CASP) within the South Liverpool Community Mental Health Team. This role involves supporting older adults with severe and enduring mental illness to re-engage with their community resources, working closely with multidisciplinary teams, primary care, and voluntary sectors. Note: Applicants must be able to travel regularly and reliably between multiple sites.

Main duties of the job
  • Reduce social isolation and loneliness by supporting social re-engagement.
  • Introduce clients to community resources and support attendance at appointments.
  • Assist with community-based activities and daily living tasks.
  • Support mental health management, including mindfulness and anxiety techniques.
  • Develop and maintain community resource networks.
  • Encourage social activities and develop social networks.
  • Facilitate peer support groups.

Detailed job description and main responsibilities

Principal Responsibilities:

  • Provide direct care and observation under supervision.
  • Report observations and assist with activities of daily living.
  • Contribute to assessments and care planning.
  • Assist with medication management and communication with other services.
  • Maintain a safe environment and support risk management.
  • Support service users in social and therapeutic activities.
  • Participate in multidisciplinary reviews and audits.
  • Support professional development and training.
Person specification
Experience
  • Experience in a care setting; mental health experience is desirable.
Knowledge and Skills
  • Effective communication, confidentiality, teamwork, and non-judgemental attitude.
  • Ability to work independently and towards Level 3 Health Care qualification.
Qualifications
  • Care Certificate, Level 2 Health & Social Care; additional desirable qualifications include phlebotomy, MEWS, and smoking cessation.
