Job overview

An exciting opportunity exists for a CHC Clinical Nurse Assessor and Discharge Facilitator at Meadow Court. The role involves supporting patient flow through inpatient rehabilitation and assessing patients for NHS Continuing Care when discharged onto the D2A pathway.

The post holder will coordinate patient discharges across Meadow Court inpatient units, requiring knowledge of person-centred assessments, discharge facilitation, mental capacity, and safeguarding procedures. Report writing skills for health needs assessments and support plans are essential.
