Job summary
This advertisement is for 3 x 37.5 hours per week posts and 1 x 22.5 hours per week post - These posts require shift work (Long days and nights are required).
Core 24 Liverpool is a specialist liaison service that supports the rapid assessment of patients mental health for those who present to the Emergency Department in times of crisis or, those who are referred from the acute general hospital wards and the formulation of treatment and risk management plans based on assessment of needs with the appropriate triage and diversion form the service.
The service operates on a shift system of long days inclusive of nights to provide 24/7 365 days a year service. The service has recently undergone Organisational Change to make the service more flexible across the Urgent Care footprint, providing support to Mersey Cares additional Core 24 services including Southport, Warrington, Aintree and Whiston Core 24 services where required.
Main duties of the jobTo provide high quality, evidence based care within Division for service users and their carers within inpatient and community services.
To work in a socially inclusive way and in line with the recovery model.
To function as named nurse/care coordinator to service users undertakingassessment, planning and evaluation and review of care, within a recovery focused model of care.
To participate as an active member of the multi disciplinary team.
To provide advice, expertise and leadership within the team.
To supervise care for service users presenting with complex and high risk needs.
To work autonomously, reaching clinically appropriate decisions based oninformation received and using the knowledge and experience of the multidisciplinary team.
To be qualified to act as a mentor for students

Where appropriate to job role take responsibility for the safe administration of medication to service users ensuring that the Trust policies and procedures and the Nursing & Midwifery Councils regulations are adhered to.
