An exciting opportunity has arisen for a Band 5 Care Practitioner to join the Out of Hours District Nursing Team. The service operates across Sefton Place from 8pm to 8am, 7 days a week. Working in line with Mersey Care values, you will demonstrate excellent empathy, communication skills, and a person-centred approach. You should be eager to continue learning and developing the competencies necessary to provide evidence-based care to patients in their own homes. Clinical decision-making skills and a problem-solving approach are essential for this role. You will be a good team player, capable of working independently, escalating issues appropriately, and reporting back as needed.
You will also have the opportunity to be mentored, with full support provided to develop and enhance your skills.
