The role of Discharge Coordinator is a key part of the MDT, providing support and specialist advice to the clinical nursing team, AHPs, medical teams, and extended MDT including social care and outside agencies. The role involves offering high-level support to facilitate the timely discharge of vulnerable and premature infants to home and family care, alongside service evaluation.
The post holder will leverage their experience to support the transition from hospital to home, working closely with community partners to ensure a safe and supportive environment, with ongoing support where needed.
Active participation in multi-professional and multi-agency meetings is expected, including providing reports and updates. The role also involves safeguarding responsibilities, integrating existing pathways into daily practice, especially those specific to the neonatal ward. The post holder will lead quality and safety initiatives, setting and monitoring standards with support from the Matron.
We offer a Band 7 Neonatal Discharge Coordinator position at 0.5 WTE in Neonatal ICU, Royal London Hospital, to join our team. We seek a motivated, experienced Neonatal Nurse to support the Neonatal team and collaborate with families to ensure seamless care transition from hospital to community, liaising with the Neonatal Community Outreach team and relevant agencies.
