Job summary

We are looking to recruit a Specialist Occupational Therapist to join our team as part of the wider Community Adult Health Service, serving the Hertsmere locality in Hertfordshire.

The position offers experience with a mixed caseload, including amputees, neurological/stroke rehabilitation, Parkinson's disease, falls/balance impairments, orthopaedics, long-term conditions, and palliative/end-of-life care. The goal is to rehabilitate and maximize patients' potential, supporting activities that promote independent living at home.

Successful applicants will be part of an enthusiastic, patient-focused, holistic team treating patients in their homes. Our multidisciplinary team includes Occupational Therapists, Physiotherapists, Nurses, Healthcare and therapy assistants, and administrative support.

We collaborate with GPs, hospitals, social care, hospices, and third-sector organizations.

It is essential that you have a full UK driving license and access to your own car to travel within the Hertfordshire area.

Main duties of the job
  1. Conduct comprehensive, multi-professional assessments and develop evidence-based treatment plans.
  2. Perform overview and complex specialist assessments as needed.
  3. Apply clinical guidelines to practice.
  4. Develop individualized treatment programs using clinical reasoning and client-centered goal setting.
  5. Manage your caseload effectively, communicating with team leads about service capacity.
  6. Prescribe and fit equipment to maximize independence, including delivery from satellite stores.
  7. Collaborate closely with multidisciplinary team members.
  8. Work independently following Lone Working policies.
  9. Identify and liaise with external agencies for patient support and referrals.
  10. Triages referrals for OT and Physiotherapy, prioritizing based on clinical need.
