Are you looking for a new challenge? An exciting 6-month secondment has arisen in the adult Haemophilia CNS team, to cover maternity leave.

The Leeds Haemophilia Comprehensive Care Centre care for over 1200 patients with a range of inherited and acquired bleeding disorders. The team also work closely with Bradford and York Centres which make the North and West Yorkshire Haemophilia Network.

The CNS team are a fundamental part of the MDT which strive to provide a high standard of care for our patients at a time where there are several developments in treatment options.

The MDT includes the medical team, CNS, physiotherapist, psychologist, specialist laboratory and admin team, there is also regular need to liaise with other teams such as orthopaedics, obstetrics, and dental colleagues.

The team is passionate about improving patient care and welcome an individual with the same goal to join us.

Main duties of the job

The role of the Haemophilia CNS is a dynamic and varied role.

Patients registered with the service have 24-hour access to the Haemophilia team for bleeding related issues and this includes a walk-in service to the purpose-built centre within the Bexley wing 08:00-17:00 Monday to Friday. The CNS team support with the assessment and treatment of patients in this setting.

The team also supports with co-ordinating treatment and care for patients undergoing surgical procedures and as LTHT has numerous sites this involves working with teams in other hospitals such as the LGI and CAH.

Home visits are also part of the CNS role providing access to care for those with bleeding disorders that have difficulty attending the hospital setting.

The CNS team also run a successful telephone clinic for those patients with milder symptoms.

The successful candidate will be supported by the established team to develop their knowledge of bleeding disorders.

The individual will also be encouraged with aspects of service development.

2. JOB PURPOSE/SUMMARY

The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.

The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.

The job holder will support the development of interdisciplinary and whole service working. As a role model they will promote the sharing of good practice and delivery of high quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers.

Leadership

Provide a visible clinical lead and role model to the nursing and wider team by delivering high quality compassionate care.

Support staff to provide efficient, effective, safe and quality care to patients as an expert practitioner.

Deliver a positive patient experience.

Support in the delivery of an effective, evidence based, safe, quality service within budgetary envelope.

Provide support to the clinical teams to develop new ways of working.

Provide effective leadership in the development of staff at all levels.

Responsible for the day to day supervision of junior clinical staff and students within the specialist team.

Motivate staff in the delivery of quality patient care.

Contribute to the recruitment and selection process.

Contribute to Nursing and Midwifery objectives and service improvement.

Supervise clinical delivery of care competencies.

Anticipate problems and take preventative or mitigating actions, or have contingency plans available.

Investigate and respond to accidents, complaints, untoward incidents and other significant events.

Maintains record of workload and performance monitoring information and reviews this on a regular basis with team manager.

Under the direction of the Team Manager participates in or lead the development of specific elements of nursing practice, reflecting the CSU and Nursing, Midwifery and Allied Health Professionals business plans.

Engage users and carers views on service delivery and development.

Contribute to and implement policy and practice guidelines specific to the clinical specialty, proposing changes where appropriate.stock control utilising equipment and supplies appropriately.

Clinical

Provide high quality care as an expert practitioner and act as a role model and resource for advice, support and guidance.

Where appropriate to the services patient demographic, to developmentally appropriate, young person friendly healthcare to support young people including support for transition, as they move from childrens into adult services between the ages 11-25 years.

Participates in the setting, monitoring and improvement of standards and quality of patient care.

Undertake clinical procedures where dexterity and accuracy are required.

Assess, plan, implement and evaluate care and treatments using specialist clinical knowledge and clinical skills.

Make clinical assessments and treatment decisions within clearly defined policies and protocols.

Organises own clinical workload.

Undertake duties on wards or departments as and when required by service demands or patient need.

Supervise clinical staff to promote the delivery of excellent clinical standards.

Receives supervision of clinical care and outcomes from nurse specialist team leader.

Act in accordance with the relevant professional code i.e. NMC,

Fulfil the requirements as directed by the Nursing and Midwifery Council to maintain and revalidate registration.

Ensure and promote the maintenance of a safe, clean and secure working environment.

Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the Leeds Teaching Hospitals Trust (LTHT) safeguarding policies.

Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes.

Ensure that documentation standards are maintained in accordance with NMC and LTHT guidelines, whether written or computerised.

Promote and maintain positive working relationships between all staff, internal and external to the Trust, involved in the care of patients.

Communication

Maintains accurate clinical and other records using the LTHT documentation.

Support patients and their families, communicating sensitive condition related information, ensuring they receive required information to enable them to participate in their care delivery.

Deliver complex information to patients and staff.

Liaise with external health providers and other agencies (as appropriate) to ensure seamless provision of care delivery.

Quality Improvement & Research

Improve the quality and safety of our services by being proactive in promoting and supporting the Leeds Improvement Method.

Contribute to quality improvement programmes as required.

Keep up to date with and communicate service developments, advances and research.

Through the use of audit, local research, metrics and feedback continually seek to identify practice that needs improvement. Implement and evaluate any resultant improvement plan.

Actively promote patient involvement and feedback within service improvements to promote a positive patient experience Maintain and sustain the momentum of improvements until they are firmly embedded into the values and culture of the clinical area.

Undertake audit programmes specific to client group, treatments and developments.

Help to set, monitor and seek to continuously improve standards and the quality of patient care, including those standards defined in local and national guidance.

Qualifications

Essential

  • Registered Nurse (RN Adult for adult clinical areas or RN Child for childrens areas (Level 1 or 2)
  • Recognised teaching/assessing qualification or equivalent experience of teaching and assessment
  • Evidence of continued professional development
  • Willing to undergo training as necessary


Desirable

  • Post registration qualification in specialty
  • Degree Level Education or evidence of working towards.
  • Nurse Prescribing qualification


Experience

Essential

  • Current issues in healthcare
  • Significant and recent relevant experience working as a Band 5 Staff Nurse
  • or equivalent post registration experience within speciality.
