This highly specialist pharmacist post is responsible for providing oversight and direct support to the virtual ward pharmacy service provided to patients supported by CHIC (Community Health and Integrated Care) division community-based teams.
The post holder will collaborate with an integrated workforce of doctors, nurses, and AHPs, as well as specialist colleagues in Respiratory, Heart Failure, and Frailty. They will work with a dedicated pharmacy technician and other members of the ESHT and CHS pharmacy teams. The postholder must liaise with colleagues working in the virtual ward, crossing both professional and organisational boundaries.
The role involves providing patient care in patients' homes and requires the ability to travel independently across sites. The use of technology to support and monitor patients is essential.
We seek an experienced, enthusiastic, self-motivated, and dynamic Pharmacist with excellent communication and interpersonal skills. The successful candidate will work with colleagues across the integrated care system to develop and maintain high-quality, clinically driven pathways to enhance patient experience.
Ideally, the applicant will hold a prescribing qualification and be keen to expand their scope of practice.
