**Schedule:** Per Diem
**Shift** : Day Shift 8am-5pm , PRN
**Location: In Homes covering Jackson and Clay Counties.**
**The role you’ll contribute:**
This position is accountable for assisting the health professional by providing personal health care services and assistance with activities of daily living to the patient as specifically identified in a written assignment and necessary for the accomplishment of patient care plan goals.
**The value you’ll bring to the team:**
+ Providing Home Health patients with assist in activities of daily living such as bathing and personal care.
+ Providing Home Health patients with assist in instrumental activities of daily living such as preparing meals, shopping for groceries or personal items, performing light or heavy housework, doing laundry, and using a telephone.
**The expertise and experience you’ll need to succeed** **:**
**Minimum qualifications:**
+ Active American Heart Association BLS
+ Valid In State Drivers License and Car Insurance
+ A minimum of ninety (90) hours of training including sixteen (16) hours of supervised practical training by a qualified RN covering the following areas:
+ Communication skills;
+ Observation, reporting and documentation of patient status and care or service rendered;
+ Reading and recording of TPR;
+ Basic infection control procedures;
+ Basic elements of body functioning and changes in body function that must be reported to the Registered Nurse or Supervisor;
+ Maintenance of clean, safe and healthy environment;
+ Emergencies and knowledge of emergency procedures;
+ Physical, emotional and developmental characteristics of the population served by the Department, including the need for respect for the patient, his privacy and his property;
+ Appropriate and safe techniques in personal hygiene and grooming, including:
+ Bed bath;
+ Sponge, tub or shower bath;
+ Shampoo, sink, tub or bed;
+ Nail and skin care
+ Oral hygiene; and
+ Toileting and elimination.
+ Safe transfer techniques and ambulation;
+ Normal range of motion and positioning;
+ Adequate nutrition and fluid intake; and
+ Other health-related topics or tasks pertinent to home health services.
+ Currently certified to practice as a Home Health Aide.
+ Successful completion of a Home Health Aide competency evaluation which addresses the following subject areas:
+ Observation, reporting and documentation of patient status and care or service rendered;
