Job overview

We are seeking an enthusiastic and passionate Advanced Clinical Practitioner (ACP) to join us for a 12-month period to support the development of our Frailty SDEC Unit & co-located Frailty Short Stay Unit.

The ideal candidate will be an experienced advanced practitioner with a non-medical prescribing qualification (NMP), capable of working autonomously with assessment, examination, diagnosis, and management skills across the frailty pathway. NHS experience is essential; applications from nursing and AHP practitioners are welcome.

This is a full-time (37.5 hours) position, working Monday to Friday. It is a fixed-term contract for 12 months to cover maternity leave, with potential for permanency.

Existing NHS/WWL staff are encouraged to apply via secondment, with prior approval from their substantive line manager.

Main duties of the job

You will be part of a skilled team managing frailty patients within the Frailty SDEC, demonstrating advanced clinical skills to assess, diagnose, and plan treatment, including ordering and interpreting investigations.

You will work autonomously, making independent clinical decisions and collaborating closely with ED and Community Response Team to ensure timely patient transfers.

  • Accountable for all aspects of your own work and patient management.
  • Provide face-to-face consultations and clinical leadership within the team.
  • Take comprehensive patient histories, perform examinations, and utilize clinical judgement for diagnosis.

  • Manage individual patient care, including assessment, diagnosis, planning, and discharge.
  • Apply advanced clinical skills and promote research and education.
  • Evaluate care quality and participate in risk management and service development.

  • Participate in ward rounds and contribute to patient management decisions.

The unit operates 7 days a week, with shifts between 0800 and 2000.
