An established non-profit organization is seeking a Part-Time Community Connector to enhance mental health services within GP practices. This role involves supporting the development of integrated care models, providing early intervention and care planning for patients, and coordinating access to community services. Established for over 30 years, they have provided services alongside a variety of statutory, voluntary, community, and social enterprise (VCSE) colleagues in Surrey and its borders. Salary: GBP30,280 per annum pro rata
Hours: 21 hours a week
Duration: 16-month FTC

This role is part of a new initiative funded by NHS England to pilot integrated primary and community mental health care models within 12 sustainability and transformation partnership areas across the country.
The Community Connector plays a key role in developing these innovative teams and services within GP practices (Primary Care Networks). Responsibilities include supporting the development and mobilization of integrated primary care mental health services, working with multidisciplinary teams including GPs, practice nurses, mental health practitioners, pharmacists, and psychologists.
You will act as the first point of contact for GPs seeking support for patients with mental health issues, providing early intervention, triage, assessment, and care planning. Additionally, you will coordinate access to community services such as social care, housing, debt advice, and employment counseling to help individuals achieve their goals.
