Job Summary

GP Primary Choice Ltd is seeking a specialist social prescriber to join their team, covering GP surgeries in the Colchester area on a full-time basis.

The role involves providing holistic advice and support to the public, linking individuals to local services, with a focus on working with patients experiencing dementia, along with their families and carers. The successful candidate will need to drive due to travel requirements. Main Duties
  1. Contribute to reducing long-term health and social care demand through proactive, holistic care.
  2. Identify patients who could benefit from this service using risk registers.
  3. Collaborate with care and residential homes to support patients and families.
  4. Work with hospitals to facilitate patient discharge back into the community.
  5. Act independently, make home visits, and assess complex information.
  6. Listen to service users, carers, and families, providing emotional support when needed.
  7. Offer advice, support, and assistance in accessing services, acting as an advocate if necessary.
  8. Discuss patient issues with GPs or primary care teams and update records accordingly.
