We have revamped our clinical pharmacy service across NHS Fife as part of an ambitious project to integrate care across hospital, primary care, and specialist teams. This initiative aims to ensure elderly care services are coordinated Fife-wide, facilitating smooth transitions and optimal patient outcomes regarding medication management.
We are seeking experienced pharmacy technicians registered with the GPhC, possessing proven post-qualification experience, to join our integrated pharmacy team working across Fife within the community health clinical pharmacy team.
You will be based at Victoria Hospital, working within supported discharge wards, the rapid assessment and discharge unit, and across Acute Services, Community Hospitals, and Hospital at Home.
Your responsibilities will include medicines reconciliation to ensure elderly patient safety, collaborating with multidisciplinary teams to support discharge flow and pathways from Frailty/Care of the Elderly wards, and providing a safe, effective pharmacy service across Fife to facilitate care transitions.
The role involves travelling across Fife to cover other community hospitals outside your base location. You will also have dedicated sessions to maintain your checking qualification.
You should be able to work independently or as part of a team, possess excellent communication skills, and have experience working in a clinical environment and managing staff.
