Organisation: Leicestershire County Council

Work Location: Room 700, Rutland Building, County Hall, Leicester Road, Glenfield, LE3 8RA

Worker Category: Hybrid Worker - Shift working. Office Base Mon - Fri AM shift, PM, weekend & Bank Holidays shift working from home.

Working Hours: 37 hours per week

Contract Type: Permanent

About the Role

1 x 37-hour Post

The HART service provides care and support under its registration as a domiciliary care service, including reablement, hospital discharge support, and short-term crisis support to promote independence and meet ongoing needs.

Key Duties
  • Be the first point of contact for all internal and external customers to the HART Service.
  • Provide comprehensive initial contact and duty functions, prioritising queries, issues, and referrals in line with response times.
  • Accept initial referrals and screen against HART Criteria.
  • Programme efficient deployment of culturally appropriate reablement support to meet goals and needs.
  • Prioritise urgent referrals to meet response times within two hours.
  • Programme Service to maximise resource utilisation.
  • Operate 365 days a year from 7am to 10:30pm with a rolling shift pattern including weekends, early, late, day shifts, and bank holidays.
About You
  • Minimum of 4 GCSEs A - C including English Language or equivalent qualifications.
  • Experience working in local government and Adult Social Care.
  • Experience managing risk and ensuring positive outcomes, with research skills for signposting queries.
  • Experience working in a team, supporting colleagues, and customer service experience.
  • Knowledge of ICT customer service databases and Microsoft Office Suite.
  • Excellent communication skills, quick learner, and accurate information processing.
  • Ability to record critical information, demonstrate quick thinking, and have good keyboard skills.

Access to a vehicle, flexible working approach, and an Enhanced DBS check are required.
