Dynamic Support Keyworker Assistant

Are you looking for a new and exciting opportunity to be part of a developing team who are passionate about supporting children and young people with a learning disability and/or autism who are at risk of admission to a mental health hospital?

As a Dynamic Support Keyworker Assistant, you'll help young people and their families navigate complex systems, coordinate support, and advocate for personalised care -- all with the aim of preventing or shortening hospital stays. You'll work closely with young people on the Dynamic Support Register across South Staffordshire, ensuring their needs and voices are front and centre in all decisions.

You'll be a trusted point of contact for families, helping them feel heard and supported throughout their journey. You'll work with services across health, education, and social care to ensure the young person's needs are met.

This is more than a coordinating role -- it's about making sure families never feel alone in the system and that each young person's voice is truly heard.

Main duties of the job
  • Participate in the regular reviews and discussions of the team's caseload.
  • Assist in the implementation of pathways of support as directed by the Keyworker Team Lead.
  • Promote independence, focus and active involvement in the keyworker service.
  • Ensure that young person receives a culturally competent service.
  • Observe and report any changes in the young person's physical, mental health and behaviour to the team.
  • Be aware of community resources and facilitate the appropriate referrals.
  • Maintain accurate and timely information in accordance with agreed standards, Trust Policies and the Data Protection Act.
  • Deliver person centred outcomes.
  • Identify any barriers to good communication such as emotional distress and hostility and overcome them through de-escalation and therapeutic communication skills.
  • Actively maintaining and updating systems including the LDA Keyworker platform, accessing resources and completing relevant training.
  • The post holder is expected to work under the supervision of the Dynamic Support Keyworker Lead. There is an expectation that they will receive managerial and clinical supervision in line with the trust policy.
  • Use professional judgment when working alone to assess complex situations where risk is present, to feed back to keyworker team.
  • Liaise with and develop positive partnerships and relationships with local stakeholders.

Person Specification
QUALIFICATIONS AND TRAINING
  • A minimum of a NVQ Level 3 / BTEC National, Certificate/Diploma, Vocational A-LEVELS/ An ONC (Ordinary National Certificate) / OND (Ordinary National Diploma) qualification in one of the below:- ochildcare, osocial work, oyouth work, ocounselling or oeducation
  • A relevant apprenticeship, such as the Early Intervention Practitioner or Children, Young People and Families Practitioner apprenticeships.
EXPERIENCE
  • Previous experience of working with children, young people and families and evidence of relevant training.
  • Experience (paid or voluntary) of working with families and children in either a health, social, education or voluntary role.
  • Demonstrable knowledge and understanding of equality and diversity issues and the ability to integrate into employment policies and practices
  • Experience of working/caring for CYP with a Learning Disability or Autism diagnosis.
  • Experience of working within a multi-disciplinary team
  • Experience of dealing with Parents in person and on the phone.
  • Experience of facilitating Multi-Disciplinary Team Meetings
SKILLS, KNOWLEDGE & ABILITIES
