Are you looking for a new and exciting opportunity to be part of a developing team who are passionate about supporting children and young people with a learning disability and/or autism who are at risk of admission to a mental health hospital?
As a Dynamic Support Keyworker Assistant, you'll help young people and their families navigate complex systems, coordinate support, and advocate for personalised care -- all with the aim of preventing or shortening hospital stays. You'll work closely with young people on the Dynamic Support Register across South Staffordshire, ensuring their needs and voices are front and centre in all decisions.
You'll be a trusted point of contact for families, helping them feel heard and supported throughout their journey. You'll work with services across health, education, and social care to ensure the young person's needs are met.
This is more than a coordinating role -- it's about making sure families never feel alone in the system and that each young person's voice is truly heard.
We welcome applicants from a wide range of backgrounds -- including: Support work, youth work, social care, advocacy, education, or mental health with lived experience of disability, neurodiversity, or caring roles.
Join a passionate, values-driven team and play a key role in transforming how children and young people with learning disabilities and autism are supported.
