Responsibilities

  • Care coordination and rota management.
  • Attendance at weekly huddle meetings.
  • Reconciliation of electronic records.
  • Conduct observations/audits.
  • Perform care plan reviews.
  • Manage daily duties and out of hours.
  • Lead peer development sessions.
  • Signpost and advise staff and clients.
  • Lead client drop-in sessions to shape service changes.

Overview

Responsibilities
  • Care coordination and rota management
  • Attendance at weekly huddle meetings

  • Allocated daily duty tasks
  • Management of out of hours
  • Peer development sessions
  • Signposting and advising staff, clients
  • Leading client drop in sessions, to shape service changes
Working Hours

Week 1:

  • Monday 9:30am‑12:30pm
  • Tuesday 5pm‑10pm (plus sleep 10pm‑7am) 1h handover 7am‑8am
  • Wednesday off
  • Thursday 5pm‑10pm (plus sleep 10pm‑7am) 1h handover 7am‑8am
  • Friday off
  • Saturday off
  • Sunday off

Week 2:

  • Monday 9:30am‑12:30pm
  • Tuesday 5pm‑10pm (plus sleep 10pm‑7am) 1h handover 7am‑8am
  • Wednesday off
  • Thursday 5pm‑10pm (plus sleep 10pm‑7am) 1h handover 7am‑8am
  • Friday off
  • Saturday responsible manager for on call contact
  • Sunday responsible manager for on call contact
