The Southend East PCN is seeking a dual qualified nurse trained in Mental Health to lead triage for acute mental health concerns. This permanent role will involve managing complex diabetes care and supporting practices in delivering high-quality patient care across the community, ensuring a holistic approach to health management.
Job summary
The Southend East PCN is looking to employ a dual qualified nurse who is trained in Mental Health. The PCN works closely with the community Mental Health provider - EPUT, and is looking to bridge the gap between community Mental Health and Primary Care Mental Health.
Main duties of the jobTo be the first contact for patients with acute Mental Health concerns for the member practices, leading on Mental Health QOF and triage.
To identify patient's diabetes care needs using a holistic approach and to assess and offer highly specialist advice in all aspects of complex diabetes management to all members of the multidisciplinary team.
Chronic disease management - assess, plan, implement and evaluate individual treatment plans for patient, including insulin initiation.
Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
About usSouthend East PCN is located at Norton Place in Shoebury, along Ness Road. We have our own PCN building which is extremely active and is in use by our 5 member practices in the local area. We cover approx 37,000 patients and offer a range of appointments from our wide range of staff, fully utilising the ARRS budget. Our team consists of pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists and assistant practitioners. We have 2 clinical directors who are partners in two local surgeries, and an operations manager to provide support wherever needed. We have a strong digital presence and have employed a Digital Transformational Lead, to help our PCN network with other businesses and mature over time.

Clinical practice
To be the first contact for member practices with their acute mental health triages. To lead and support the member practices with their Mental Health QOF.
To work closely with the Community Mental Health Team - EPUT, and be a part of the complex care MDT meetings, involving many different partners of the Southend East Integrated Neighbourhood Team
Chronic disease management - assess, plan, implement and evaluate individual treatment plans for patient, including insulin initiation.
Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
To demonstrate a positive supportive approach to patients at diagnosis and/or patients with the development of diabetes related complications.
Work with patients in order to support adherence to prescribed treatments
Establish and maintain highly effective communication skills with patients, carers/relatives, and professionals across health and social services. This may involve the giving of sensitive and complex information.
On occasion the post holder will be required to visit patients in their own home e.g. to conduct chronic disease reviews.
Perform advanced clinical skills in assessment, diagnosis and treatment.
Assess, plan, implement and evaluate care delivery according to changing health care needs.
Collect, collate, evaluate and report information, maintaining accurate patient records.
Involve patients and carers/relatives in the planning and delivery of care and development of services.
Work collaboratively with the multidisciplinary team and outside agencies to ensure patient needs are met, especially in relation to ongoing care needs
Act as a resource for specialist advice and expert clinical knowledge across the Practice, giving advice to colleagues on the clinical management of patients.
Provide expert specialist advice and education to patients and carers/relatives to enable the safe and independent management of their diabetes.
Undertake clinical supervision and education of nursing colleagues on an individual or group basis.

Recognise and work within own competence and professional code of conduct as regulated by the NMC
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

Assess effectiveness of care delivery for the nursing team through peer review, benchmarking and formal evaluation
