Job description

This is your opportunity to be a part of a new and exciting DualDiagnosis Service, this new service is provided in conjunction with ourcolleagues at We Are With You. The post holder will help to develop and deliverdual diagnosis services, ensuring that high quality services are delivered toservice users with complex diagnosis. The post holder will work alongside theother members of the dual diagnosis team to offer evidenced based interventionsto service users in the community. The post holder will assist the dualdiagnosis Lead to develop and maintain high quality standards of care.

Main duties of the job

The successful candidate will be part of a joint project between Navigo and With You to provide dual diagnosis support for those who are experiencingboth mental health and substance issues. The team work along side a small butvaried MDT inclusive of a Dr, Nurse, Recovery Workers, Peer Support and SupportWorkers. The team support people with a variety of tasks inclusive ofsignposting, emotional support, harm reduction, substance advance and onwardreferral to support people to be empowered within their lives and support themto achieve their set.

Job responsibilities

1. To supportthe development and implementation of treatment within the field of dualdiagnosis (mental health and substance use/misuse) for service users identifiedas potentially having a dual diagnosis.

2. To provideone to one sessions with service users, ensuring that care and recoveryplanning in relation to Substance misuse is conducted on an individual basisaccording to the specific needs of the service user.

3 Toestablish, sustain and disengage from therapeutic relationships withindividuals.

4. Tointeract and co-operate with service users, relatives, and other professionalsand any community services as appropriate.

5. To haveprofessional responsibility for promoting equality, diversity, and rights.

6. To workclosely with ward-based nursing teams and multidisciplinary teams to supportthe implementation of substance misuse care plans

7. Toimplement and monitor treatment plans with service users that address theirneeds in relation to mental health and substance misuse problems.

8. To assessand act upon immediate risk of danger to service users and/or others.

9.. Toeducate and give information to service users and others in the care of theclient on issues related to mental health problems and substance misuse.

10. Tosupport service users to engage in interventions aimed at addressing theirsubstance misuse difficulties.

11. Able todeliver structured psychosocial interventions including harmreduction/minimization strategies, motivational interviewing, CBT programmes,problem solving, behavioural interventions and counselling techniques asnegotiated with individual service users. (Training provided).

12. Toco-facilitate and support the delivery of group programmes throughout Navigo'sin-patient services and within the community.

13. Whenrequired to advocate for service users.

14.Where appropriate, to engage friends and family of the service user in theirtreatment

Person Specification
Knowledge
  • Ability to work independently.
  • Willingness to learn.
  • Car driver/ability to travel to different sites.
Qualifications
  • Training in vocational work, career advice or equivalent experience gained through work in the field.
  • An awareness of the role of CPD and Reflective practice in effectively working in this setting.

Experience
  • Experience of working in health, social care or related setting.
  • Experience of contributing to multidisciplinary teams risk assessment and discharge planning processes
  • Have the ability to build and maintain effective relationships with clients and colleagues alike
  • Be able to work effectively in a team, supporting others when needed.
  • Be resilient under pressure and able to prioritise work
  • Have both oral and written communication skills
  • Be comfortable using IT
  • Be organised and able to manage your time effectively
  • Able to be flexible with working hours on occasion.
  • Awareness of the Mental Health Act and Mental Capacity Act and its significance for service users and impact on clinical practice.
  • Knowledge of evidence based interventions to support people who are recovering from mental illness
