Job summary

To provide a supportive role to the Out of Hours Community Nursing Service in delivering high standards of healthcare to patients within their own homes, care homes, the travelling community, and in clinics.

Main duties of the job

As part of the Urgent Community Response (UCR) Team, you will be required to:

  1. Work in partnership with the nurse co-ordinator to support deployment of clinical teams as directed.
  2. Drive nurses to patient visits.
  3. Undertake care under the supervision of the registered nurse.
  4. Engage in the use of SystmOne, including mobile working.
  5. Engage with referral to discharge standard processes.
  6. Be aware of the demand and capacity model reflecting workload needs at any given time.
  7. Engage in admission avoidance when required.

Job description

Job responsibilities

Coordination support

  • Work as part of the clinical triage team within the Out of Hours Community Nursing service, supporting referral registration and visit allocation under the supervision of a nurse coordinator.
  • Ensure documentation and recordkeeping comply with SOPs and Trust policies.
  • Use assistive technology to support fair and efficient visit deployment.
  • Assist with urgent referral processing using geographical knowledge and technology.
  • Follow local processes and SOPs.
  • Troubleshoot assistive technology alongside the nurse co-ordinator.
  • Communicate effectively with deployed teams.

Driver

  • Locate and drive the Out of Hours Nurse safely to designated addresses and back.
  • Maintain vehicle fuel, logs, and safety regulations.
  • Interact with satnav devices, including troubleshooting.
  • Communicate effectively with the coordination team.

HCA

  • Assist nurses with moving and handling patients and clinical tasks.
  • Assist with equipment delivery and collection.
  • Follow policies on consent and infection control.

General

  • Maintain patient confidentiality and dignity.
  • Contribute to service delivery and support communication within teams.
  • Report problems and incidents appropriately.
  • Ensure vehicles and equipment are used and maintained safely.
  • Contribute to supply requisitions and manage indirect patient contact tasks.
  • Coordinate shift coverage with the team.
