Job summary

Qualifications
- RN Adult with District Nurse qualification or relevant experience.
Responsibilities
- Lead a defined caseload within the community nursing team.
- Ensure quality of care and achievement of financial and performance targets.
- Provide clinical supervision and line management to the team.
- Collaborate with GP practices and other professionals to ensure consistent care.
- Work flexibly across core hours and weekends as required.
