District Nursing Lead - Advanced District Nurse Specialist

We are pleased to offer a career development opportunity within the District Nursing Service for District Nurse Specialists aiming to progress to a senior, advanced clinical, and leadership role.

We welcome applications from those nearing the completion of their District Nurse Specialist Practitioner Qualification.

Main duties of the job

You will develop and deliver the preventative care agenda, providing clinical leadership for the local development of the national ageing well anticipatory care model in Oxfordshire, working closely with senior leaders. You will foster relationships and integrated working with multi-professional teams to implement the model in local populations.

You will ensure that planned care within District Nursing teams aligns with quality standards and evidence-based pathways, aiming for optimal patient outcomes and effective caseload management.

You will lead the DN teams in managing patients with more complex and/or advanced needs.

You will work collaboratively to provide leadership and support for the locality DN teams alongside the District Nurse Lead and the District Nursing Locality Lead.

Job responsibilities

Oxfordshire District Nursing Service is innovative and involved in national initiatives to improve community nursing services to meet future demands.

Career progression within the service is well established and continues to develop, especially around senior clinical and leadership roles.

The service operates within Planned and Preventative Care under the Community Services Directorate, delivering care to housebound and other patients in their homes, clinics, and residential care homes.

Salary: £46,148 to £52,809 per annum, pro-rata where applicable.
