Central and North West London NHS Foundation Trust in Hillingdon is seeking a qualified District Nurse Team Leader to cover maternity leave from November to July 2027. This Band 7 role requires DNSPQ and substantial community nursing experience.

You will lead a district nursing team, manage a GP-attached caseload, and drive development of district nursing services within an integrated care setting, with support from senior nurses and a collaborative multidisciplinary team.

Formación

• Currently registered with NMC as an RGN.
• DNSPQ qualification and extensive community nursing experience.
• Degree level or higher qualification.

Responsabilidades

• Provide management support and professional nurse leadership to all members of the nursing team.
• Develop, implement and review new initiatives involving District Nursing services.
• Manage a GP-attached clinical caseload of housebound patients.
• Lead staff through change to embed service improvements.
• Coordinate integrated care with specialist teams and GP practices.
