Overview

Surrey Downs Health & Care invites applications for a Band 7 Community Nursing Team Lead role (full-time, 37.5 hours/week), permanent, based in Leatherhead PCN. The successful candidate will join an established community nurse team working closely with the wider multidisciplinary team across the PCN and Surrey Downs Health and Care. Summary of the Band 7 roles for Leatherhead PCN: the District Nurse Team Lead and Community Nursing Team Lead are Band 7 leadership positions responsible for delivering high-quality, person-centered care in the community. Core duties are identical: leading and managing a community nursing team, overseeing assessment, planning and implementation of care, ensuring staff development, maintaining professional standards, and contributing to clinical governance and risk management, with emphasis on integrated care, workforce planning, change management, resource management, and participation in KPIs and audits. Both roles emphasize respect, collaboration, safeguarding and high delivery of care to patients and staff.

Responsibilities
  • Lead within the District Nursing Team and be responsible for the assessment of patient needs and the development, implementation and evaluation of a programme of care within agreed standards.
  • Facilitate educational programmes for basic, post basic and in-service training, in close liaison with the Learning and Development Team.
  • Lead and facilitate patient-focused, innovative, research-based practice in line with Surrey Downs Health and Care objectives.
  • Promote integrated care, support workforce planning, manage change, manage resources and participate in KPIs and audits.
  • Demonstrate clinical expertise and act as a professional role model to all staff, internal and external, as part of the Surrey Downs Health and Care team; develop services in line with patient needs.
  • Respect patient privacy and dignity; be accountable for assessments, care planning and appropriate prescribing; involve, support, inform and educate family/carers.
  • Provide specialist knowledge and advice to influence the SDHC strategic agenda.
  • Maintain clear, comprehensive, signed and contemporaneous records according to procedures.
  • Identify workforce planning issues and participate in recruitment, selection and retention of clinical staff.
  • Provide support and a learning environment for pre and post registration students as required.
  • Ensure policies, procedures and standards of care are adhered to at all times.
  • Manage budgets within remit efficiently.
  • Participate in research and development opportunities as appropriate.
  • Assist with resolution of complaints within the clinical specialty or area of responsibility.
  • Negotiate and work effectively across agencies to maximise care effectiveness.
  • Be aware of and implement safeguarding procedures to protect vulnerable individuals as necessary.
  • Lead and implement the Surrey Downs Health and Care Clinical Governance Strategy; facilitate clinical audits and monitoring of care.
  • Lead and implement the SDH&C Risk Management Strategy; ensure all processes are followed.
  • Maintain professional and clinical integrity in line with NMC guidelines.
  • Undertake any other duties consistent with the post; ensure timely data entry for self and corporate teams.
  • Be prepared to work within Surrey Downs Health and Care localities and move base temporarily or permanently as service needs require.
