Job Summary

Are you looking for opportunities to develop your leadership skills and be part of a dynamic Community Nursing Team within Harrow?

The Community Nursing Teams in Harrow are seeking Band 7 District Nurse Team Leaders responsible for providing clinical leadership and managing individuals within the team to deliver high-quality community care.

Main Duties
  1. Manage care of patients referred by GPs, aiming to prevent unnecessary hospital admissions through assessments, care planning, and coordination with other agencies.
  2. Communicate regularly with GPs, participate in meetings, and plan care and treatments to reduce hospital admissions.
  3. Maintain clinical credibility by providing high standards of evidence-based care and applying clinical leadership to improve practice.
  4. Collaborate with the Clinical Development lead to organize and develop training programs for District Nurses.
