Job summary

Sanctuary Personnel is seeking a Band 5 or Band 6 District/Community Nurse in Guildford, Hampshire. This ongoing role offers flexible hours and both full-time or part-time contracts, with an hourly pay of £22.00 to £28.00.

You will conduct holistic assessments and develop care plans in patients’ homes, delivering wound management, catheter care, medication administration, and palliative care. Travel to Haslemere, Guildford and Waverley areas may be required.

Qualifications

• Registered Nurse (Adult) with active NMC registration.
• Band 6 requires district nursing qualification or specialist community qualification.
• Band 5 requires practical experience from pre-registration or post-registration training.
• Must be able to drive to sites around Haslemere / Guildford / Waverley.

Responsibilities

• Conduct holistic assessments, develop care plans, and evaluate patient care in homes and community settings.
• Deliver clinical interventions including wound management, catheter care, medication administration, and palliative care.
• Manage and prioritise an assigned caseload and respond to evolving clinical needs.
• Participate in community response and admission avoidance initiatives to keep patients at home.
• Provide travel to Haslemere, Guildford, and Waverley areas when needed.
