Would you like to work in an integrated team? Would you like to ensure patients are discharged safely into the community following a hospital stay? Do you have excellent communication and IT skills?

The discharge hub at West Middlesex Hospital was set up initially as a response to the COVID-19 pandemic. The discharge hub brings together West Middlesex Hospital, Hounslow and Richmond Community Healthcare and Adult Social Care to ensure patients are discharged in a safe and timely manner

We are looking for individuals who enjoy working as part of a team in a complex and fast changing environment. You will be responsible for supporting the clinicians in the discharge hub to ensure patients are transferred in a safe and timely manner. You need to be a great communicator who pays attention to detail and works in a methodical way.
