The Discharge Flow Coordinator plays a key role in maintaining hospital flow and discharge processes. Working in a regional, system-wide role, the discharge coordinator operates under the direction of the Urgent and Unscheduled Care Programme Management and collaborates with ward and multidisciplinary teams (MDTs) across Argyll and Bute.
The role involves planning and implementing patient discharges (both operational and service development) and facilitating patient flow through the hospital by:
- Driving effective and timely transfer of patients from hospital to appropriate settings
- Reducing delays in patient transfers from hospital
- Communicating effectively with patients, carers, and case managers to involve them in discharge planning from admission onwards
- Supporting discharge planning with guidance from therapists as needed
- Liaising with doctors, discharge services, social services, care teams, and other MDT members for proactive discharge planning
- Identifying and escalating concerns related to discharge or delayed transfers through established structures
Main duties of the job
- Act as a facilitator attending board rounds, patient/family meetings, and liaising with social and therapy services
- Organize patient case conferences and meetings independently, especially in complex cases
- Collaborate with ward MDTs and the Hospital Discharge Service to deliver high-quality discharge planning aligned with patient and family needs
- Promote a multidisciplinary approach and involve patients and carers in discharge decisions
- Monitor discharge pathways and escalate issues when deviations occur
- Provide relevant, accurate, and timely information to patients and families to support informed discharge decisions
- Update electronic patient records as required
- Serve as a single point of contact for oversight and assurance of the delayed discharge programme
