A healthcare organization in Croydon is seeking a full-time case manager to facilitate effective patient discharges. The role requires strong communication skills and critical thinking to work within a multidisciplinary team, ensuring patients are supported through their discharge plans. The candidate should hold a clinical or social work qualification and have experience in the acute hospital setting.
The post holder will be responsible for case managing and facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate oversee Discharge Navigators within their sub-team, complete assessments of patients needs and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Transfer Of Care Hub Team (TOCH) to overcome and limit these.
The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.
This role will extend to support and ensure that patients who have been discharged feel supported and have received the support that was requested. You will need to have critical thinking skills and be able to communicate effectively. This role will be full time across 7 days so flexibility will be required.
