Overview
We are looking for a highly motivated and enthusiastic individual to work closely with our Matrons, Clinical Nurse Manager and Charge nurses to facilitate the timely, effective and appropriate discharge of patients from the acute setting to the community, liaising with hospital, community, social and health service colleagues in order to promote a recovery focused discharge/transfer. The post holder will also play a key role in developing an innovative service providing excellent discharge practices within the ward environment.
Responsibilities
- Ensure best practice in planning safe and timely patient discharges to achieve the best outcomes for patients, optimise hospital bed utilisation and reduce length of stay.
- Communicate effectively with internal multidisciplinary teams, senior staff, ward staff and Allied Health Professionals, and liaise externally with Social Services, Primary Care Services, General Practitioners, Housing, Homeless Persons Unit, Department of Health, Borders Agency and Office of the Public Guardian.
- Promote a smooth discharge process by focusing on tasks that support the multidisciplinary team in safe and timely discharge; complete discharge planning documentation; communicate with external agencies such as Social Services, patients’ families and carers; arrange appropriate transport; liaise with Occupational Therapy, Physiotherapy, Speech and Language Therapy, Dietitian, Clinical Psychologist and Doctors.
- Where appropriate, provide access to patient homes to outside agencies, accompany patients to their homes or to appointments with outside agencies.
- Monitor and chase MDT members and external agencies to promote timely and safe discharge, provide optimum bed usage and reduce length of stay; manage more complex discharge issues.
- Prepare weekly SitReps to enable accurate reporting of Discharge To Cause (DTOC) to the Department of Health.
- Issue Discharge Notifications to Social Services to ensure reimbursement guidelines are observed.
- Promote wellbeing and safety of patients and carers by acting promptly on concerns, communicating effectively and sharing information according to policy and procedures.
- All posts working with vulnerable groups will be subject to an enhanced criminal records check on appointment; staff will consent to regular updates on their criminal records status throughout employment.
Requirements / Qualifications
- Excellent communication and organisation skills; ability to work under pressure.
- Experience in discharge planning, liaison with multidisciplinary teams and external agencies preferred.
