Job summary

Job description

Working within clinical areas, the post holder will facilitate safe patient discharges, providing a high-quality service in line with Trust standards. They will communicate sensitively and effectively with a wide range of people, including patients, families, colleagues, and external agencies, to support patient discharge.

Main duties of the job

The Discharge Coordinator will manage patient flows within a defined area, prioritise work, review practices to ensure timely and efficient patient access to Trust services, and oversee the patient administration pathway from admission to discharge.

Experience
  • Experience working in a healthcare environment or care setting

Qualifications
  • 5 GCSEs A-C including English and Math, or equivalent (Level 4 Diploma/NVQ/BTEC)
