An Integrated Care System in Leeds is seeking a Discharge Coordinator to facilitate safe patient discharges and provide high-quality services in line with Trust standards. This role involves managing patient flows, communicating effectively with patients and healthcare professionals, and ensuring compliance with Trust policies. Candidates should have experience in healthcare and possess strong literacy, numeracy, and IT skills, along with a minimum qualification of 5 GCSEs including English and Math.
Working within clinical areas, the post holder will facilitate safe patient discharges, providing a high-quality service in line with Trust standards. They will communicate sensitively and effectively with a wide range of people, including patients, families, colleagues, and external agencies, to support patient discharge.
The Discharge Coordinator will manage patient flows within a defined area, prioritise work, review practices to ensure timely and efficient patient access to Trust services, and oversee the patient administration pathway from admission to discharge.
