An exciting opportunity to join IUCT Acute as a Discharge Case Manager.
You will be responsible for undertaking timely nursing assessments and Fast track discharges., liaising with Continuing health Care. Supporting the safe and effective discharge planning of patients from acute wards into the community. Completing Home First assessments.
Starting discharge planning on admission and supporting ward staff with complex discharges. ensuring safe and timely discharges
To complete Nursing assessments and Fast track Discharges.
Supporting ward staff with complex discharges
Completing Home First referrals
Supporting End of Life patients to be discharged
Liaising with IUCT Community
