Job summary

University Hospitals Dorset NHS Foundation Trust is seeking a Band 6 Nurse or Occupational Therapist to join the Integrated Discharge Team, supporting discharge planning and the D2A model across Poole and Bournemouth sites. The role requires autonomous practice within a multidisciplinary team and promotes Home First principles to ensure timely discharges.

The post is based at Royal Bournemouth or Poole Hospital with weekend and Bank Holiday shifts as the service operates seven days a week.

Responsibilities

• Work with the Integrated discharge team in the Transfer of Care Hub to achieve effective discharge planning.
• Act as a discharge expert autonomous practitioner supporting the MDT to facilitate safe, timely patient discharge.
• Promote Home First and Discharge to Assess principles to avoid delays and manage expectations.
• Provide timely discharge for patients not meeting clinical criteria to reside.
• Advise patients, families and staff on discharge planning processes and SOPs.
• Record discharge information accurately on ward tracking systems for real-time communication.
