Job summary

Hurley Group is recruiting for a PCN Digital Transformation Manager within North Lambeth PCN, serving approximately 76,000 patients across six member practices. You will lead the strategy, liaise with staff and system stakeholders (local councils, Social Services), drive adoption of new technology to improve access and productivity, and foster collaboration across practices to deliver enhanced care.

Responsibilities

• Develop and deliver the PCN Digital Transformation Strategy in liaison with staff and system stakeholders (e.g. local councils / Social Services).
• Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.
• Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients, including shared appointments between practices to aid delivery of enhanced access.
• Lead an improvement approach to change, including building capability for quality improvement within the PCN and system-wide approaches to problem solving.
