The post holders will work with clinical and operational teams within Primary Care Network (PCN) NOTTINGHAM EAST to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and deliver changes in a collaborative way. The post holders will support this vision to be developed in alignment with ICS strategy.
The post holders will line manage the PCN Personalised Care Workforce and progress the integrated neighbourhood working, Community Health hub, and health inequalities projects within the PCN.
The post holders will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement, and then support these to be delivered.
The post holders will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.
The post holders will develop relationships with ICSs to gain agreement and support for local transformation plans and activity, and to support alignment with broader strategy, including the ICS’s digital strategy.
Working with relevant colleagues in the ICS, the post holder will also develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support effective delivery of transformation.
The post holders will work with the ICS to request specialist IT and estates expertise required to deliver transformation plans, in line with the ICS’s delegated responsibilities for GPIT and GP estates.
The core objective of the Digital & Business Transformation Manager is to support the PCN, its practices, and the PCN workforce to deliver ongoing improvement to services.
The role will require experience in business/service improvement with expertise in transformational change and a good understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvement.
This expertise will be used to develop and implement a strategic plan to build and implement a robust digital and business infrastructure.
Nottingham City General Practice Alliance (NCGPA) was formed in 2016. We are Nottingham City's first GP Federation, often referred to as NCGPA or the Alliance. Our members consist of over 40 NHS GP practices in Nottingham city, who cover a population of over 336,000 patients. We are the lead employer for our PCN based roles such as Clinical Pharmacists, Pharmacy Technicians, Social Prescribing Link Workers, and Health and Wellbeing Coaches.
