Job Summary

Are you passionate about using digital innovation and data to transform healthcare and tackle health inequalities? Do you want to make a real impact in your local community? Clacton Primary Care Network is seeking an experienced and dynamic digital and transformation lead to join our team, working across Clacton PCN practices and community settings, with some remote working possible. The role is for30 hours per week with flexible working considered.

This pivotal role will lead transformation and improvement initiatives across Clacton PCN, supporting integrated neighbourhood teams and driving innovative models of care. Working closely with our member practices and partner organisations, you'll champion digital enablement, population health management, and service redesign - all aimed at improving patient outcomes and access.

Main duties of the job

  • Develop and implement a transformation plan aligned with local and national priorities.
  • Lead service redesign projects that reduce health inequalities and improve patient care.
  • Champion digital tools that enhance patient experience and workforce efficiency.
  • Use population health data to drive targeted interventions.
  • Promote inclusive engagement with patients and underserved communities.
  • Support PCN workforce development in digital skills and quality improvement.
