Reports to:GeneralTeam Leader
Risk Assessment:The post involves loneworking when patients are seen, for privacy reasons in an outpatientconsultation room and when alone in the Department office.
Job Purpose:
To manage a caseload of patients offeringevidence-based dietetic advice and/or patient centred principles withindesignated ward areas and in out-patient clinics.
To provide dietetic intervention to patients on awide range of therapeutic diets in a timely way by working as a member of thedietetic team and other health care professionals.
To take responsibilityfor prioritising and managing workload, seeking guidance from senior staffwhenever required.
To train and updatehealthcare professionals on nutrition related topics.
To contribute to theTherapy Clinical Governance agenda to further improve the quality of theservice.
To provide dieteticadvice to patients at 3 hospital sites including group education sessions tohelp deliver a responsive dietetic service provision.
Key Duties:
Clinical
Tofunction as an autonomous practitioner, working within codes of practice andprofessional guidelines when providing expert advice to patients on therapeuticdiets.
Touse clinical judgement and analyse the patient's personal circumstances beforeformulating a dietary care plan.
Toaccept clinical responsibility for a caseload of patients and organise thiseffectively.
Toprioritise caseload of inpatients and outpatient referrals received to ensuretimely dietetic intervention.
To advise patients inindividual consultations on therapeutic diets in Outpatient clinics or viatelephone and to inform referrer in writing of dietetic intervention.
To liaise with cateringdepartment regarding special dietary requirements
Toinvolve patients in decision- making for their treatment on wards and inoutpatient clinics; to inform referrer in writing of dietetic intervention.
Toformulate and update diet sheets on specific topics to ensure information isevidence based
Toadvise patients in group education sessions
Tocoordinate a safe and effective discharge/transfer of patients from hospital
Toattend Departmental meetings and participate in on-going development of theservice.
Toensure patient confidentiality is maintained at all times.
Communications
Tocommunicate effectively with the multidisciplinary team, patients carers andrelatives regarding the dietetic component of patient
Tomaintain accurate documentation on-going patient care, including phone contactsin accordance with professional standards.
Tocommunicate with healthcare professionals in implementing Trustwide nutritionrelated policies.
Towork with healthcare professionals to help improve dietetic care and patientexperience.
Touse computer skills to produce and retrieve clinical information, tocommunicate to colleagues and for presentations.
Educationand Training
To maintain own clinical professionaldevelopment
To be appraised annually and agree objectivesfor the year and a personal development plan taking into account personalobjectives.
To participate in student training programmes inthe Department following attendance on a clinical supervisory skills course.
To contribute to multidisciplinary trainingprogrammes linked to nutrition in the Trust
To participate in clinical supervision.
Organisationand Management
Toassist teams with day-to-day planning and delivery of patient caseload.
Tocontribute to the pro-active monitoring and review of the Nutrition &Dietetics service.
Governance
Clinical:
To contribute to the implementation of ClinicalGovernance in the Department.
To carry out clinical audit and present resultsto promote evidence-based practice.
Non Clinical
To implement Health & Safety in theworkplace by maintaining a safe environment for patients, staff and visitors atall times.
To act immediately to rectify unsafe practices.
To report adverse incidents to staff, patientsor any other person according to Trust policy.
To attend mandatory training ensuring there isadequate cover for clinical work.
To adhere to all Trust and departmentalpolicies and other statutory requirements such as Infection Control,safeguarding procedures.
Professional
To maintain Health Care Professions Council(HCPC) registration, adhering to the Standards of Conduct Performance andEthics.
To maintain own competency to practice throughCPD activities
To work within the British Dietetic Association(BDA) code of conduct.
