A unique opportunity to join an exciting project has arisen within a pilot programme on Anglesey which supports the health and well-being of children and families.
The successful candidate will work alongside fellow family support workers under the guidance of the Senior Project Coordinator, delivering and expanding support services to families. This will involve testing new ways of working to engage families in the home as well as the delivery of community-based interventions, utilising a range of agreed approaches to encourage positive changes to food, nutrition and physical activity behaviours.
The post holder will work relatively independently within an agreed framework with a degree of autonomy, responsibility and accountability for tasks. This will include responsibility for a caseload, delivery of support to groups of families and working co-productively with a range of partners and organisations in a complex community environment.
The post holder will support the project lead to monitor and quality assure interventions and contribute to the evaluation of outcomes and sustainability of approaches tested.
