Job summary

Reset Health is seeking a Dietetic Associate Practitioner (DAP) to deliver protocol-led nutrition and behaviour change support for adults with obesity and related long-term conditions, under supervision of a registered dietitian. You will work within the Wraparound pathway, support patient education, follow-up, documentation, and escalation of concerns, contributing to safe, scalable digital care.

Schedule: Full-time (40 hours/week).

Qualifications

• Foundation Degree or Level 4 health qualification or equivalent experience.
• Experience supporting patients in healthcare, community or wellbeing settings.
• Experience delivering patient education or behaviour change interventions.
• Experience working as part of a multidisciplinary team.
• Knowledge of nutrition, healthy lifestyles and person-centred care.

Responsibilities

• Deliver protocol-led lifestyle, nutrition and behaviour change support for patients within designated pathways.
• Conduct structured patient follow-up and reinforce care plans.
• Maintain accurate clinical documentation in line with standards.
• Collaborate with Senior Dietitians and MDT to support coordinated patient care.
