An exciting opportunity has arisen for an enthusiastic, motivated, and experienced nurse with excellent communication skills to join the Diabetes Team. We are recruiting a part-time Diabetes Transition Specialist Nurse, working closely with age groups between 16 and 25 years. You will collaborate with Consultants, Clinical Nurse Specialists, and other multidisciplinary team members to deliver care and management for people with diabetes in both community and acute settings. This role is based between Bentley Medical Centre and Manor Hospital. You must be able to work autonomously, support this population across Walsall, organize your workload effectively, and be adaptable and proactive in driving change. The ideal candidate will be a registered nurse or a registered children’s nurse with extensive post-registration experience. A postgraduate qualification or experience in diabetes clinical care is preferred.

Walsall Healthcare NHS Trust is an integrated provider of NHS acute care, serving a population of 284,300. It offers inpatient and outpatient services at Manor Hospital and various community services. The hospital has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and an Integrated Assessment Unit. The new Urgent and Emergency Care Centre, opened in March 2023, has expanded emergency care capacity and facilities.
Provide care in both acute and community settings for people with diabetes, including clinics in GP surgeries, health centres, and homes.
Work within a multidisciplinary team alongside consultants, nurses, podiatrists, dietitians, and educators.
Deliver patient and healthcare professional education sessions in line with service agreements.
