Job Role

Diabetes Support Nurse

Band 5 - £29,970 - £36,483
37.5 Hours per week
12 months temporary contract

Responsibilities

An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Locala Diabetes Service.

We are seeking a Band 5 Diabetes Support Nurse with the ability to make a difference to the health and wellbeing of individuals referred to the service. The role requires a motivated individual interested in developing themselves and others to deliver high-quality, effective, and innovative diabetes care in the community.

You will need to demonstrate knowledge of national and local issues and be committed to partnership working with other providers.

As part of a team working in conjunction with GPs, mainstream nursing, social, and secondary care services, you will provide a patient-focused, coordinated case management approach across primary care.

The post holder will participate in and influence efforts across health and social services to shape the service, support patient choice, improve quality of life, and promote self-management, adopting the community care team approach.
