Job description

An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Locala Diabetes Service.

We are seeking a Band 5 Diabetes Support nurse who can make a positive impact on the health and wellbeing of individuals referred to the service.

Main duties of the job

The role requires a motivated individual interested in developing themselves and others to deliver high-quality, effective, and innovative diabetes care in the community. The candidate should demonstrate knowledge of national and local issues and be committed to partnership working with other providers.

As part of a team working with GPs, mainstream nursing, social, and secondary care services, you will provide patient-focused, coordinated case management across primary care. The post holder will participate in and influence efforts across health and social services to shape the service, support patient choice, improve quality of life, and promote self-management, adopting a community care team approach.
